Fees
I work in a direct and personal relationship with my patients with regards to their personalized care as well as with financial reimbursement. This means that payment is expected at the time that service is given. You can request a receipt/superbill with procedure codes and diagnoses that you may submit to your insurance provider directly for reimbursement. The exceptions to this are those persons injured in a motor vehicle accident that have PIP (personal injury protection) insurance and those persons injured in a work-related injury and are covered by Labor and Industries (L and I) coverage. Medicare patients can receive preventive/maintenance care but as Medicare services are not billed, for acute spinal care services, we are happy to refer you to local providers who will treat and bill Medicare for Medicare covered services.
15-minute Consultation to discuss treatment options: No Cost
New Patient History/Exam and Initial Treatment: $150 – $200 depending on time and complexity
30-minute Appointment: Personalized treatment that may include adjustments, exercises, EFT stress reduction, etc: $75

First time appointments with Dr Craig are 60 minutes in duration.
Follow up appointments are 30 minutes in length so that you can have a truly individualized one on one session with Dr Craig.

If you are unable to print and complete them prior to your visit, please contact the doctor so that arrangements can be made to either have the forms emailed or mailed to you or time can be arranged for you to arrive early to complete them.
